On the GIA report, are the measurements given just for the actual size of the table, or the actual spread of the top of the diamond - including the table and the girdle?

The GIA report will give the minimum and maximum diameter of the diamond x depth percentage as well as the table percentage. It will show the length x width x depth for fancy shapes.

Actually, it's the length x width measurement I'm referring to for fancy shapes. Does that mean the entire top of the diamond from edge to edge, or is it just the table?

GIA gives the length and width of the entire stone. The table percentage for fancy shapes is the relationship of the table across the WIDTH of the stone only. No reference is made or measured against the length:table ratio at all.

If you multiply the table percentage times the width measurement you will have the mm measurement of the table width. Not that you can do much with it, but it there it is.
