Is there a way to tell, such as on the grading report, where a diamond came from (read: mined), say South Africa, Russia, Canada, etc?

Mine came with a "birth certificate", lol! It is an Isee2 stone, and they all come with one. It was kind of interesting to read about it!
 
Unless the diamond comes with an audit trail, there is no way to tell - not like we can via inclusion suites with say ruby or sapphire.
Some diamonds in different continents grew in the same place and were transported up thru the mantle via different volcanic eruptions on diferent continents, e.g. South America and Central Africa were once joined and the diamonds there came from the same source

Except for some Canadian diamonds that are processed in accordance with government regulations on Canadian origin, no, full stop. And if you don''t trust those paper trails (some don''t), there is no other way.

Much of it has to do with diamond geology. Diamonds form deep in the mantle, which does not provide geologic clues one could use to track a stone back to a specific mine on the surface.
