When did H & A come around? Do older stones have H & A?

Do old, well-cut stones exhibit hearts and arrows? (I guess OECs and OMCs don''t, but I''m asking about standard RBs)
When did this style of cutting begin (year?)

I believe I heard some time in the mid 1990s . . . for some reason 1998 sticks in my head but I could be wrong.
