Looking to have custom locket made

Anybody know the best route I should take? I'm thinking I want a heart shaped 14k or so white gold locket with "MAXX" written on it in small diamonds. Are there some online companies that might excel in something like this? Or would my best bet be to try and find the best jeweler around me? I want it to be put on a bracelet, so I'm not sure what the best size is.. I'm thinking maybe 3/4 inch? (input on the best size lockets for bracelets would be good too)

Also, any idea what something like this would cost?
