How common are loose stones/setting issues?

Just out of curiosity, how common is it that a stone will become loose in a setting, or that you may loose a stone all together (main, sidestone, etc?). After having our ER for about a year and a half and not long after getting it back from having our custom wedding band made, we noticed the center store was loose. Of course I was told that any little shock to the setting could cause this but seeing that we hadn't even been married yet, I was pretty OCD about keeping it in good condition after we got it back with the wedding band since I needed that ring to look nice in pictures! My mother, who just celebrated her 37th anniversary and has a lovely solitaire in a 18k gold setting, has not once needed that thing repaired. Perhaps it's just luck, but maybe I'm just naïve in not realizing that this is a normal occurrence! Please set me straight!
 
I would not say it is normal. It can happen, of course, when there is impact to the head of the ring. But the only time I had a loose stone in 30 years was when a prong caught on something and pulled away from the stone (but it was still there held by the other prongs). Are you careful to take the ring off when doing things like working out, doing dishes, housework, etc.? Of course, even doing that cannot prevent accidental hitting against something, and sometimes we are not even aware when it happens. I guess there are also occasions where a stone wasn't set correctly, but I think you would have had problems sooner if that was the case.
 
My ering I lost one of the trillion side stones. My eternity band don't even want to go there as i lost 5 I finally had it reset from platinum to 18k white gold we thought that the platinum was too soft but guess what lost 2 stones since its been in 18k white gold.
