I have Octaviaitis.

I want to sell a diamond, or two, or three.
I want a safe place to exchange the diamond for the cash.
Also having it appraised will not only protect the buyer (I know, not MY problem) but it will also protect me from being accused of misrepresentation and swapping.

I'd like to meet the buyer at the office of a mutually-agreed upon appraiser.
After appraising the diamond the three of us will meet at appraisers facility.
Appraiser presents report to buyer.
Buyer hands cash to appraiser.
Appraiser hands me cash and hands buyer diamond.

Do you appraisers here offer this service?
Is there an additional fee for this 5-second, but valuable, service?
Are there factors I have not mentioned that I should also consider?

Better Idea:

You offer these terms:

1) Let Buyer Choose Appraisor.
2) You do due diligence on Appraisor (Accrediation they have a safe and insurance etc.)
3) If okay you send to appraisor like a dealer would with terms, only to be released upon your written notice.
4) If buyer likes diamond and appraisal you meet buyer at appraisors office or buyer arranges for payment and appraisor only releases diamond upon your consent.
 
I have appraised and then allowed the deal to be consumated at my office many times. The appraisal fee comes to me and the parties can discuss the terms for a few minutes and make the exchange. I don''t want any part of the money end of the deal, but maybe other appraisers occasionally handle that part, too. Believe me, this is no 5 second service. A lot of coordinating and patience needs to be used to make such deals take place. They can be tricky at best and have a potential for danger, too. How do you know who is going to show up? Will they come to rob you? Are they a crook? You have to take risks with such deals.

Having the diamond graded before the deal by GIA or AGS would be the best way to represent it. Ask a price far enough below Pricescope range that the deal makes sense to the kind of person looking for the risky bargain you are offering. Meet at the office of an appraiser who is willing to have you use his/her facility and pay the appraiser for the process required. Sometimes these deals go very smoothly and everyone gets the right price.
 
I do this sort of deal fairly regularly where the buyer and seller meet in my office. In such cases my client is the buyer, not the seller. I charge for the appraisal regardless of whether the deal goes through and no matter what the asking or final transaction prices are. I do the inspection with both present, then send the seller to a waiting area down the hall with the ring in hand and I go over the observations and report with the buyer in private. The buyer then goes out and negotiates with the seller and, if they decide to make a deal, I bring them both back into the lab, inspect the piece to be sure that nothing has changed and they do their deal under my cameras. If they can’t make a deal they both go on their merry ways.

I would not describe this as an escrow service as you have in the headline but I think it's basically what you're looking for and I agree that it protects both buyer and seller. To be a true escrow, the appraiser takes possession of both the ring and the money and then passes things on to the correct people when a contract is agreed to. This is necessary if one or both customers aren't local and yes, I charge a fee for it above and beyond the appraisal when it comes up. This is not a popular service.

Are you trying to do this as a business? (One or two or three diamonds?) Or are you trying to cut your losses on a few stones? I have facilitated these types of deals several ways depending on the type, quality, condition of the stone and motives of the seller. The first step is usually involves sending the stone to GIA for a report. It helps to level the playing field.
It is not a "five second service". If it was easy, everyone would do it.
