I think they are for cartilage, nose etc. piercings.

Yes! The first time I saw something like this was when I got my second hole pierced two years ago. I absolutely love them because they’re so comfortable for sleeping in. You can put them in your lobe piercing as well as a cartilage piercing. Called labret studs. Can be internally threaded (screw in) or threadless (a push pin). I prefer threadless. When you first get them, you have to bend the post a little bit so that it clicks into the back. There should be instructions that come with the earrings, but that was some thing I had to learn. Yes. They are for cartilage piercings but lots of people put them in their earlobes. They are a slightly bigger gauge — I cannot put them in my lobes.

You could find something like this at a higher end body piercer in gold (some body piercers have/use gold jewelry...these are basically a cheaper version of body jewelry). I've heard of Maria Tash or Royale Body Jewelry making more high-end body jewelry and you might be able to order right from their website. You might also find something similar on Etsy.

They look like La poucette backings which are spring loaded and are made for earlobe piercings. They fit with most earring backs and don’t require a thick gauge of earring post. You can buy these backings independently and use them with any stud earrings.

Yes, I have a pair of 3mm purple CZ in titanium settings like this in the 2nd piercing I had put in January of this year. My partner had 4mm fake opal and titanium settings like this with the flat backs used for their first ear piercing.

They are quite comfortable to sleep in. I have to remove my diamond studs every night for a variety of reasons (comfort, not wanting to loosen the prongs on the setting, etc.), but these I leave in no worry.
