Any opinions on if this is costume or real? It has been in the family (Britain) for over 110 years. All opinions are welcome ... thanks

I am guessing paste, but could easily be wrong. A jeweler could probably tell right away, though not diagnose any treatments.

It is not really possible to tell from photographs with any certainty and as Boerumbiddy said, it will be a good idea to have a gemologist take a look at it. The stone does look sufficiently included so that someone could take a look and give you an opinion.

ETA: I hope you wear it and enjoy it!

It looks to be an early paste piece, but with those photo's and that much facet wear to the crown it is near impossible to tell.
A competent jeweller should be able to tell at a glance if viewed in person.
