Custom-Made Slip Covers?

I am looking for a place/online vendor/etc. that can help me with a custom-made slipcover. We have a chair and a half that has some odd dimensions and needs to be covered, but I want to get it made with a specific printed fabric.

Any suggestions on a place or website that would do a good job with something like this?
