Never guaranteed. First, you have to sue the labs for it in writing. Second, it seems like a coin flip as to when it's granted.

Indeed, the diamond was graded D-Flawless in that case, but HPD's client was made well-aware that only IF was guaranteed. Flawless seems to be awarded according to "lab-lotto."
