Thanks so much to everyone that helped me along in this process. I ended up doing everything through a private jeweler and they did a fantastic job.

Band: 10 5-point channel set rounds (G-I color & VS1-VS2 clarity), platinum, soltice head with a 4 prong low base setting, size 6
