Who gets invited to the rehearsal dinner?

Who gets invited to the rehearsal dinner? I have heard out of town guests and the bridal party.But, who is considered an out of town guest?People from out of state or people who have to drive far enough to want a hotel?Some people seem to be getting hotels even when they only live an hour or so away . . .

What is the etiquette with this?

We invited OOTers that had to fly in and bridal party. There were about 45 and we had a rockin'' good time!

But you can do just bridal party if you want, I don''t know that there are any specific etiquette rules on this, but I''m no expert.
