Does this look funny? I started to print out a few like this and then I second guessed the "____ of ____ will not attend" part, like it may not have to be there?

Oh and we would fill out the # of seats reserved and the second blank in the "___ of ___" part also before we sent it out.

I would dump the "will not attend" line, the rest looks great.
