I have an old ring, that my grandma gave, its gold with one cushion cut stone, ruby, I want to get it resized, Im size 6 and the ring is the biggest size there is, probably its 10 or more. I went to a jeweler, and was told that the stone needs to be removed in order to resize the ring.

My question is,
Is it common to remove stone, in order to resize a ring down?
What are the chances that such a specific ring that has specific color and cut, can be replaced with a fake one?

The way you describe the stone, I wouldn''t be a bit surprised if it wasn''t a synthetic stone already. They are very common, really. It makes little sense to remove a stone to size a ring smaller most times, but if it a large change in size there is a liklihood the prongs will be moved and the stone will become loose anyway. Be very sure you confirm the genuine or synthetic nature of the ruby BEFORE you leave it with anyone you don''t trust 100%.
