I'd like to get a pair of sapphire earrings for my wife's birthday. One pair she likes are these from Harry Winston:

I realize HW products can be marked up pretty aggressively. If I wanted to have a pair of earrings like these custom-made, a few questions for the experts here:
- What would be the likely cost of the stones and workmanship?
- How would I ensure I get stones which are similar in quality to those HW would use? I know how to do this for diamonds, but not for sapphires.
- Would the final product likely look as nice as the HW piece, or not quite as nice?
- Are there any jewelers you would suggest for this kind of work?
