You purchase white sapphires at a number of online vendors. Have you looked? Otherwise, I assume many jewelers could assist...I used to live there, but have no real suggestions.
You also might want to focus on dimensions instead of carat weight if you already have a setting that will only fit a certain size stone.
A 1 carat sapphire will not be the same size as a 1 carat diamond...
I was just out in Santa Clara, CA and I know Joe Escobar is located out there in Campbell, CA; the craftsmanship is amazing. They are pricey but they have some beautiful colored stones.
Personally, I would ask a precision cutter to source and cut one for me. The excellent cut will help the stone to look gorgeous. Gene Flenigan of PrecisionGem.com, Dan Stair of CustomGemStones, Barry of ACStones all come to mind: they are great to work with, have good sources for rough and do fantastic work.
